Why is Methadone controlled in Japan?

Methadone is a Schedule I narcotic. Japan distinguishes between chronic-pain use (permit-allowed) and substance use disorder treatment (prohibited from import) — uniquely strict among major countries.

How to apply for the NCD import permit

  1. 1Apply for the NCD import permit at least 14 days before your arrival date.
  2. 2Doctor's letter (dated within 3 months) must list: your full name, diagnosis, medical necessity, drug name, strength, daily dose, and the prescribing doctor's signature.
  3. 3Attach photos of original packaging showing drug name and strength.
  4. 4You must carry the import certificate with you and present it at customs — not in checked baggage.

Can I bring methadone for opioid use disorder treatment to Japan?

No. Japan prohibits import of methadone when used for substance use disorder treatment. If you are in a methadone maintenance program, work with both your prescriber and a Japanese clinic well in advance — interruption of treatment without medical supervision is dangerous.

Methadone for chronic cancer pain — allowed?

Yes, with an NCD import permit. The doctor's letter must clearly indicate the indication is pain management.
